Job summary

CVS Health is seeking a Case Management Coordinator to telephonically and face-to-face assess, plan, implement, and coordinate case management activities, evaluating medical needs to support member wellness.

You will develop proactive care plans, review claims, and collaborate with interdisciplinary teams to optimize outcomes and transitions to relevant CVS/Aetna programs in a compliant, patient-centered manner.

Qualifications

• 2+ years of experience in case management, behavioral health, social services, or related field
• Willing and able to travel 50-75% to meet with members face to face in surrounding areas
• Valid Driver's License, preferably in IL

Responsibilities

• Assess and coordinate all case management activities to evaluate medical needs and facilitate overall wellness.
• Develop proactive action plans to improve short- and long-term health outcomes and wellness.
• Review prior claims to assess potential impact on current case management and eligibility.
• Consult with supervisor and team to overcome barriers and present cases at conferences for multidisciplinary care.
