Qualifications

• Bachelor's degree in behavioral health or human services (psychology, social work, marriage and family therapy, counseling)
• 2 years experience in behavioral health, social services or related field
• Managed Care experience preferred
• Ability to travel within a designated geographic area for in-person case management activities
• Excellent analytical and problem-solving skills
• Effective communications, organizational, and interpersonal skills
• Proficiency with standard corporate software applications (MS Word, Excel, Outlook, PowerPoint)
• Independent work capability

Responsibilities

• Evaluate members to determine needs/eligibility and plan care
• Identify high-risk factors and coordinate care planning
• Coordinate and monitor care plan activities
• Collaborate with managers and physicians to improve outcomes
• Escalate quality of care issues through proper channels
• Engage members to support independence and healthy choices
• Document care using case management and quality processes
• Support team meetings and peer mentoring
