Qualifications

• Must reside in southern Illinois and surrounding counties.
• Minimum of 2 years of experience in case management, behavioral health, social services, or related field.
• Willing and able to travel using own vehicle 50-75% of time to meet members face to face in surrounding areas.
• Valid Driver's License, preferably in IL.

Responsibilities

• Assess and coordinate all case management activities to evaluate medical needs and wellness.
• Develop proactive plans to improve short- and long-term outcomes and overall wellness.
• Apply clinical judgment to reduce risk factors and barriers in care planning.
• Review prior claims to assess impact on eligibility and transitions to relevant programs.
• Consult with supervisor and participate in case conferences for multidisciplinary care.
