Job summary

CVS Health is seeking a Case Management Coordinator to support members, providers and community partners across the country. You will evaluate needs, coordinate care plans and help members access benefits and services to improve health outcomes.

The role requires a bachelor’s or master’s in behavioral health or human services, strong analytical and interpersonal skills, and the ability to travel locally. Work-from-home flexibility is available with a comprehensive benefits package.

Qualifications

• 2 years of case management, discharge planning experience preferred.
• Experience in behavioral health, social services or related field (2 years).
• Managed Care experience preferred.
• Ability to work from home and collaborate with teams virtually.
• Ability to travel locally for in-person activities as directed.

Responsibilities

• Evaluate member needs and eligibility; recommend care plan approach.
• Coordinate and monitor assigned care plan activities.
• Identify high-risk factors; refer to clinical case management as needed.
• Collaborate with health professionals to improve care quality and outcomes.
• Provide coaching and support to empower member decision-making and self-management.
• Engage in team meetings and peer mentoring to support quality care.
