Job summary

CVS Health is seeking a Case Management Coordinator in Illinois to telephonically and/or face-to-face assess, plan, implement, and coordinate case management activities for members to evaluate medical needs and promote overall wellness.

The role requires a Bachelor’s or master-level clinician degree (behavioral health or human services preferred), 2+ years of case management experience, and 50–75% travel with a personal vehicle. Full-time with competitive benefits and potential bonus eligibility.

Qualifications

• Residence in southern Illinois and surrounding counties.
• Minimum 2 years of case management or related field.
• Willing and able to travel 50-75% of time with own vehicle to meet members face to face in surrounding areas.
• Valid Driver's License, preferably in IL.

Responsibilities

• Assess and coordinate case management activities to evaluate medical needs and facilitate wellness.
• Develop proactive course of action to improve short- and long-term outcomes.
• Coordinate with Aetna programs and plans as needed.
• Assess member work capacity and functional limitations.
• Consult with supervisor and discuss cases at case conferences.

Education

Bachelor's Degree or non-licensed master level clinician required, preferably with a degree either being in behavioral health or human services
