Responsibilities

Assess, plan, implement and coordinate case management activities for members.
Evaluate medical needs and support overall wellness.
Use clinical tools and data review to assess needs and eligibility.
Refer to clinical resources and coordinate with Aetna programs and plans.
Review prior claims to understand impact on case management.
