Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR, First Aid certifications.

Responsibilities

• Manage patient and family caseload across assessment, admission, planning and evaluation phases of the nursing process.
• Gather data on physical, psychological, social and spiritual needs and initiate appropriate interventions.
• Coordinate care with the attending physician and interdisciplinary team for optimal outcomes.
• Provide regular visits to reassess needs and update the plan of care.
• Document care and maintain records, progress notes, and care plans timely.
• Supervise Home Health Aide delivery of care per guidelines.
