Job summary

Ambercare Hospice in Silver City, NM seeks a compassionate Weekend RN On-Call to join our team. You will provide expert clinical care to patients and families, ensuring comfort, dignity, and quality of life in a community-focused setting.

Qualifications

• Requires graduate from an accredited registered nursing program.
• Licensed RN in the state of practice (NM).
• Current CPR and First Aid certifications required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation.
• Obtain data from patient and family considering physical, psychological, social, and spiritual factors to tailor care.
• Coordinate with attending physician and interdisciplinary team for optimal care.
• Regularly visit patients to reassess needs and update the care plan.
• Provide death pronouncement support and family assistance.
• Maintain accurate charts, progress notes, and timely care plans.
• Supervise Home Health Aide delivery of care as required by policy.
