Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data from patient and family factors and coordinate with the interdisciplinary team to meet needs.
• Initiate communication with the attending physician and other agencies for coordinating optimal care.
• Provide regular visits to reassess needs and update the care plan.
• Maintain accurate patient records and plan of care timely.
• Supervise and document delivery of care by Home Health Aide as required.
