Qualifications

• Graduate from an accredited nursing program and licensed RN in the state.
• Current CPR and First Aid certifications required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assume primary responsibility for patient and family caseload using the nursing process.
• Assess physical, psychological, social, and spiritual needs and coordinate care.
• Communicate with physician and interdisciplinary team to coordinate optimal care.
• Provide regular home visits to reassess and update the care plan.
• Perform clinical pronouncement of death and support families.
• Maintain accurate records and care plans; supervise Home Health Aide as needed.
