Job summary

Ambercare Hospice is seeking a compassionate Weekend RN On-Call Nurse to join our Espanola, NM team. You will provide expert clinical care to patients and families to ensure comfort and quality of life while supporting the interdisciplinary team.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, admission processing, planning, implementation, and evaluation of nursing care.
• Obtain data and assess information from patient factors that may impact needs and coordinate with the interdisciplinary team.
• Initiate communication with the attending physician and other agencies for coordinating optimal care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records and prepare care plans timely.
• Supervise and document delivery of home health care by aides as required.
