Qualifications

• Must be a licensed RN in the state of practice.
• CPR and First Aid certifications current.
• Experience in hospice or home health is a plus.

Responsibilities

• Manage patient and family caseload across the nursing process (assessment, planning, implementation, evaluation).
• Coordinate care with physicians and interdisciplinary team for optimal outcomes.
• Perform regular patient visits to reassess needs and update care plans.
• Provide compassionate end-of-life care and support to patients and families.
• Maintain accurate patient records and documentation; supervise Home Health Aide deliveries as required.
