Ambercare Hospice is seeking a compassionate Weekend RN On-Call Nurse to join our team in Silver City, NM. You will provide expert clinical care and support to patients and families in this rewarding role, helping ensure comfort, dignity, and quality of life.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed RN in the state of practice.
• Must possess current CPR and First Aid certifications.
• Knowledge of quality assurance procedures.
• Valid driver’s license and proof of insurance.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, admission processing, planning, implementation and evaluation of the nursing process.
• Obtain data from patient, family and factors that may impact care and coordinate with the interdisciplinary team.
• Initiate communication with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, and timely care plans.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.
