Job summary

21st Century Home Health Services in California is seeking a Part time Registered Nurse Case Manager for the Redwood City-Sunnyvale area, covering Saturdays and Sundays. You will conduct SOC/OASIS assessments, perform home visits, and coordinate with physicians to deliver high-quality patient care in the home setting.

OPs include medication reconciliation, wound care, and monitoring patient status, with autonomy and flexible scheduling.

Qualifications

• Current and unrestricted CA RN license.
• BLS/CPR certification is required.
• Valid California driver's license required.

Responsibilities

• Conduct SOC admissions and OSIS assessments.
• Conduct follow ups and evaluation visits in territory.
• Perform home visits to patients in designated territory.
• Reconcile medications, wounds, treatments and procedures.
• Monitor patient conditions and report changes to Clinical team.
• Collaborate with physicians and agency staff to ensure quality care.
• Document observations and interventions for patient care management.
