Job summary

21st Century Home Health Services is seeking a Part-time Registered Nurse Case Manager for the Sacramento County territory. The role requires delivering high-quality in-home assessments and care planning on Saturdays and Sundays, with flexibility for additional weekdays.

Responsibilities include SOC admissions, ongoing visits, IV infusions, medication reconciliation, and collaboration with the clinical team to coordinate patient care.

Qualifications

• CA RN license in good standing.
• BLS/CPR certification required.
• Valid CA driver's license.
• Experience conducting SOC/OASIS is highly desired.
• Clinical knowledge in home health care.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Perform follow-ups and evaluation visits in the territory.
• Make home visits to patients in designated territory.
• Reconcile medications and manage wound care/treatments.
• Monitor patient conditions and report changes to clinical team.
• Collaborate with physicians and agency staff to deliver care.
• Initiate IV medication infusions safely per protocol.
• Document observations and care management.
