Job summary

21st Century Home Health Services is seeking a Part-time Registered Nurse Case Manager to serve the Antioch area. You will perform Start of Care assessments, SOC admissions (OASIS), and follow-up visits within an agreed territory.

Expect home visits, medication reconciliation, wound care, and IV infusions as part of the role. The position requires an active CA RN license, BLS/CPR, and a valid CA driver’s license.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired.
• High level of clinical knowledge in home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• Previous experience with IV Medication infusions highly preferred.
• Proficiencies initiating intravenous (IV) therapy is a plus.

Responsibilities

• Utilize clinical skills to complete initial patient SOC assessments and determine home care needs.
• Conduct SOC admissions and OASIS documentation.
• Perform follow-up and evaluation visits in designated territory.
• Make home visits to patients in designated territory.
• Reconcile medications, wound care, treatments, and procedures.
• Monitor patient conditions and report changes to the clinical team.
• Collaborate with physicians and staff to deliver quality care.
• Document observations and interventions for patient care management.
• Initiate and administer IV medications safely per protocols.
