Job summary

21st Century Home Health Services is seeking a Part-time Registered Nurse Case Manager to support patients in San Mateo County. You’ll perform the initial SOC assessment, conduct follow-up visits, and make in-home visits within the territory to determine home care needs and coordinate with the clinical team.

The role requires a CA RN license, BLS/CPR certification, a valid CA driver’s license, and SOC-OASIS experience is highly desired.

Qualifications

• Current and unrestricted CA RN license required.
• Current BLS/CPR Certification required.
• Valid CA Driver's License required.
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired.
• High level of clinical knowledge in home health care is highly preferred.
• Experience with IV Medication infusions is highly preferred.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Conduct follow ups, evaluation visits as needed in designated territory.
• Make home visits to patients in designated territory.
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures.
• Monitor clients’ conditions, reporting changes to Clinical team.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Initiate and Administer IV medication infusions safely and effectively.
• Accurately document observations, interventions, and evaluations.
