Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager to perform start of care assessments and follow-up visits in patients' homes within Sacramento County. The role requires weekend work with Saturday and Sunday full days, and may include additional weekdays.

You will conduct SOC admissions, coordinate care with physicians, administer IV medications as per protocols, document observations, and ensure high-quality in-home care delivery while collaborating

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
