Job summary

21st Century Home Health Services is seeking a Part-time Registered Nurse Case Manager to cover Antioch and surrounding cities. You will visit patients in homes, perform SOC/OASIS assessments, and coordinate care with physicians and the clinical team.

We value autonomy, flexible scheduling, and opportunities for advancement. A CA RN license, BLS/CPR, and a CA driver's license are required to succeed in this role.

Qualifications

• CA RN license required.
• BLS/CPR certification required.
• Valid CA driver's license required.
• SOC/OASIS experience highly desired.
• Experience with high acuity patients preferred.

Responsibilities

• Perform Start of Care (SOC) admissions and OASIS.
• Conduct follow-up and evaluation visits in the territory.
• Make home visits within Antioch and surrounding cities.
• Reconcile medications and manage wound care.
• Monitor patient conditions and report changes to clinical team.
• Collaborate with physicians and staff to deliver quality care.
• Administer IV medications infusions safely per protocol.
• Document observations and care interventions.
