Job summary

21st Century Home Health Services is seeking a Part time Registered Nurse Case Manager to cover the Solano County area. You will perform SOC admissions, home visits, and ongoing evaluations in patients' homes, coordinating care with physicians and the care team.

You will reconcile medications, manage IV infusions, and document care observations while ensuring high-quality outcomes. Requires CA RN license, BLS/CPR, and a valid CA driver’s license; weekend availability is needed.

Qualifications

• CA RN license and unrestricted practice in California.
• Active BLS/CPR certification.
• Valid CA driver's license and reliable vehicle.
• Experience conducting SOCs, OASIS is highly desired.
• Strong clinical knowledge in home health care and high-acuity patients preferred.
• Ability to work with an interdisciplinary team and manage caseloads.

Responsibilities

• Conduct Start of Care (SOC) admissions and OASIS assessments.
• Perform follow-up visits and in-home evaluations in the Solano County territory.
• Visit patients at home, document observations, and coordinate care with physicians.
• Reconcile medications, wound care, IV infusions, and other procedures.
• Monitor patient status and report changes to the clinical team.
• Collaborate with the care team to ensure high-quality outcomes.
