Job summary

21st Century Home Health Services is seeking a Part-time Registered Nurse Case Manager to cover the Walnut Creek area, conducting SOC admissions and OASIS assessments, with required home visits and IV therapy where appropriate. Weekend availability is required, with potential for additional weekday coverage.

You will document care, monitor patient status, collaborate with the clinical team, and ensure compassionate, high-quality care in patients’ homes across the territory.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting SOC/OASIS assessments is highly desired.
• Experience with IV therapy infusions is a plus.
• Excellent interpersonal, leadership and organizational skills.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Conduct follow ups and evaluation visits in the designated territory.
• Make home visits to patients in the Walnut Creek area.
• Perform duties including reconciliation of medications, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to the Clinical team.
• Collaborate with physicians and agency staff to deliver high-quality care.
• Document observations, interventions, and evaluations in patient records.
• Initiate and administer IV medication infusions safely per protocols.
