Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager to cover the Greater Sacramento Area. You will conduct SOC admissions (OASIS), follow-up visits, and home visits to patients, collaborating with physicians and the clinical team to ensure high-quality care.

The role requires CA RN licensure, BLS/CPR certification, and a valid CA driver’s license. Experience with high-acuity patients and IV infusions is a plus, with strong interpersonal and

Qualifications

• CA RN license required.
• BLS/CPR certification required.
• Valid CA driver's license.
• Experience conducting assessments and SOC/OASIS is highly desired.
• High level of clinical knowledge in home health is highly preferred.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Conduct follow ups and evaluation visits in designated territory.
• Make home visits to patients in designated territory.
• Perform assigned duties including reconciliation of medication, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to the Clinical team.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Initiate and administer IV medications safely in accordance with protocols.
• Accurately document observations and interventions in patient records.
