Job summary
21st Century Home Health Services is seeking a part-time Registered Nurse Case Manager to provide high-quality in-home assessments and ongoing care in Solano County. You will perform Start of Care (SOC) admissions, follow-ups, and in-home visits within a 5-15 mile radius, collaborating with physicians and the clinical team.
The role requires CA RN license, BLS/CPR, valid CA driver's license, and experience with SOC/OASIS is highly desired.
Qualifications
- Current and unrestricted CA RN license.
- Current BLS/CPR certification.
- Valid CA Driver's License.
- Experience conducting SOC/OASIS assessments is highly desired.
- High level of clinical knowledge in home health care is preferred.
Responsibilities
- Start of Care (SOC) Admissions - OASIS.
- Follow-ups and evaluation visits within the territory.
- Make home visits to patients in the designated territory.
- Reconcile medications and perform wound care, treatments, and procedures.
- Monitor client conditions and report changes to the clinical team.
- Collaborate with physicians and agency staff to deliver high-quality care.
- Initiate and administer IV medication infusions per established protocols.
- Accurately document observations, interventions, and evaluations.
