Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to join our interdisciplinary home care team. You will coordinate care plans, educate patients and families, and perform skilled nursing assessments across our service area.

In this role, you’ll manage complex wound care, PICC lines, IV infusions, and follow-ups, working with LPNs, HHAs, PTs, MSWs and more. IL RN and BLS certification are required; auto insurance and driver’s license are needed for field visits.

Qualifications

• IL RN license valid with current BLS certification.
• At least 1 year of nursing experience.
• Driver's license and auto insurance required for field visits.

Responsibilities

• Coordinate and provide direct/indirect patient care using the nursing process.
• Perform OASIS assessments for admissions/recertifications.
• Manage complex wound care including wound vac, IV infusions and drains.
