Job summary

Carle Health is seeking a Home Care Registered Nurse / Field Case Manager to join our interdisciplinary team. You will coordinate care plans, educate patients and families, and perform skilled nursing assessments across multiple patient populations.

The role requires strong case management capabilities, wound care expertise, and collaboration with LPNs, HHAs, PTs, and MSWs to ensure high-quality home health services.

Qualifications

• Active IL RN license required.
• BLS within 30 days of hire.
• Valid driver's license and auto insurance.

Responsibilities

• Coordinate and provide direct and indirect patient care using nursing process.
• Perform complex wound care, PICC line care, and catheter management.
• Document accurately and communicate with care team for seamless transitions.
