Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and deliver skilled nursing care in home settings across central Illinois. You will perform nursing assessments, educate families, and manage complex wounds, central lines, and infusion services.

This role requires an RN license, BLS, and a valid driver’s license with auto insurance. Collaboration with an interdisciplinary team is essential.

Qualifications

• IL RN license required
• BLS certification
• Driver's license and auto insurance
• 1 year nursing experience

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform skilled nursing assessments and care in home settings.
• Complete admission, transfer and discharge documentation in a timely manner.
• Perform complex wound care including wound vacs and dressings.
• Manage central lines including PICC and Hickman catheters and blood draws.
• Communicate with physicians and care team to update patient plans of care.
