Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to join our interdisciplinary team. You will coordinate care plans, educate patients and families, and perform skilled nursing assessments across multiple counties.

This role offers autonomy with 24/7 clinical backup and flexible scheduling to support optimal patient outcomes. Qualifications include an IL RN license, BLS, a college diploma in nursing, and at least 1 year of nursing experience.

Qualifications

• IL RN license and BLS certification required.
• 1 year nursing experience preferred.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform complex wound care and monitor central lines/PICC lines.
• Complete admission documentation, OASIS assessments, and care plan updates.
• Communicate with MD, care team, insurers, and families to coordinate services.
