Job summary

Carle Health is seeking an experienced Home Care RN/Field Case Manager to deliver skilled nursing care in patients' homes across our service area. You will educate families, coordinate interdisciplinary care, and perform comprehensive assessments and care planning with a focus on independence and wellness.

Qualifications

• IL RN license and BLS required.
• Experience in home health or field case management preferred.
• Ability to coordinate care plans and document care using nursing process.

Responsibilities

• Coordinate and provide direct/indirect patient care in home settings.
• Perform skilled nursing assessments, education, and care planning.
• Manage wounds, PICC lines, drains, and catheter care per policy.
• Complete admission, recertification, and discharge documentation timely.
