Job summary

Carle Health is seeking a Home Care Registered Nurse/Field Case Manager to coordinate and provide direct and indirect patient care across multiple counties. The role involves skilled nursing assessments, wound care, PICC line management, and collaboration with an interdisciplinary team to ensure optimal patient outcomes.

Qualifications include IL RN licensure, BLS certification, a valid driver’s license and auto insurance, with at least 1 year of nursing experience.

Qualifications

• Certifications: IL RN, BLS 30 days, Drivers License, Proof Auto Insurance.
• Education: College Diploma Nursing.
• Work Experience: 1yr nursing experience.

Responsibilities

• Coordinates and provides direct and indirect patient care using the nursing process.
• Performs complex wound care including wound vacs, wound management and dressings.
• Monitors and manages central lines (PICC/Hickman) and ports; collects blood samples and infusions as needed.
• Performs OASIS assessments and admission documentation within policy guidelines.
• Collaborates with interdisciplinary team and coordinates care plans across patient populations.
