Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to join our interdisciplinary home care team serving diverse counties. You will coordinate care plans, conduct skilled nursing assessments, and educate patients and families in their homes.

The role requires an active IL RN license, BLS, a valid driver’s license, and 1 year of nursing experience. You’ll work with LPNs, HHAs, PTs, and social workers to ensure safe, quality care across counties.

Qualifications

• Illinois RN license required with current status.
• BLS certification obtained within 30 days of hire.
• Valid driver's license and auto insurance.
• College diploma in nursing; at least 1 year of nursing experience.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform complex wound care, wound vac, and catheter care.
• Manage PICC lines, blood draws, and IV infusions.
• Collaborate with the health care team to plan and monitor care.
• Complete OASIS assessments and admission documentation within policy.
