Job summary

Qualifications

• IL RN licensure is required.
• BLS certification within 30 days of hire.
• Valid driver license and auto insurance.

Responsibilities

• Coordinate and provide direct/indirect patient care using nursing process.
• Perform skilled nursing assessments and education for patients and families.
• Complete admission paperwork and OASIS/485 changes within policy timelines.
• Supervise Home Health Aide and coordinate care with interdisciplinary team.
