Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and deliver skilled nursing care across multiple counties in central Illinois. You will operate as part of an interdisciplinary team, ensuring patient needs are met through detailed case management, assessments, planning, and direct care in patients' homes.

Responsibilities include complex wound care, PICC line management, IV infusions, and supervising Home Health Aides, with a focus on promoting independence and wellness

Qualifications

• IL RN license required.
• BLS certification required within 30 days of hire.
• Driver's license and auto insurance required.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform OASIS assessments for admissions, recertifications, resumptions and discharges, within policy timelines.
• Demonstrate complex wound care, wound vacs, PICC line care and IV infusions per agency protocol.
• Monitor central lines (PICC/Hickman) and perform dressing changes as required.
• Direct and supervise Home Health Aide staff and coordinate care with the interdisciplinary team.
