Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and deliver skilled nursing care in the home setting across multiple counties. You will work with an interdisciplinary team, perform OASIS/485 changes, and manage complex wound care, PICC lines, and IV infusions.

Candidates must have IL RN license, BLS, 1 year nursing experience, and a valid driver’s license. The role offers a supportive environment with 24/7 clinical backup and mileage reimbursement.

Qualifications

• IL RN license required with BLS certification.
• Minimum 1 year nursing experience in home health care.
• Driver's license and auto insurance proof required.

Responsibilities

• Coordinate and provide direct and indirect patient care using nursing processes.
• Perform OASIS assessments and manage admissions, recertifications, and discharges.
• Supervise Home Health Aide and coordinate care with the interdisciplinary team.
• Manage complex wound care including wound vacs and catheter care.
