Job summary

Carle Health is seeking a Home Care Registered Nurse / Field Case Manager to join the interdisciplinary home care team in central Illinois. You will coordinate care plans, perform skilled nursing assessments, educate patients and families, and manage complex wound care, PICC lines, and catheters in patients’ homes.

The role requires an IL RN license, BLS, a driver’s license, and at least 1 year of nursing experience.

Qualifications

• Current IL RN license required.
• BLS certification within 30 days of hire.
• Valid driver's license and auto insurance proof.
• Minimum 1 year nursing experience in home care or related field.

Responsibilities

• Coordinate and provide skilled nursing care to patients in home settings.
• Perform OASIS/485 changes within required timelines.
• Supervise Home Health Aide and coordinate interdisciplinary team.
• Document thoroughly and communicate with physicians and care coordinators.
