Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and provide direct and indirect patient care in home settings across Illinois. You will work with an interdisciplinary team including LPNs, HHAs, PTs, MSWs, and others to plan and implement care, with emphasis on wound care and infusion services.

The role requires an IL RN license, BLS, a college nursing diploma, and at least 1 year of nursing experience.

Qualifications

• IL RN license and BLS certification (within 30 days)
• College Diploma in Nursing required
• 1 year nursing experience preferred

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform OASIS assessments for admissions, recertifications, resumptions and discharges.
• Manage complex wound care, including wound VAC, PICC lines and catheter care.
• Collaborate with interdisciplinary team and coordinate case conferences as needed.
