Job summary

Carle Health is hiring a Home Care RN/Field Case Manager to join an interdisciplinary team delivering care across central Illinois. You will coordinate care plans, educate patients and families, and perform skilled nursing assessments in patients' homes.

The role requires IL RN licensure, BLS within 30 days, a valid driver's license, and at least 1 year of nursing experience. You will collaborate with LPNs, HHAs, therapists, MSW, and physicians to ensure high-quality home care.

Qualifications

• IL RN license required.
• BLS certification within 30 days of hire.
• Valid driver's license and proof of auto insurance.
• 1 year nursing experience preferred.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform OASIS assessments and complex wound care as needed.
• Supervise Home Health Aide and coordinate care with the interdisciplinary team.
• Ensure documentation compliance and timely communication with physicians and care coordinators.
