Job summary

21st Century Home Health Services in Northern California seeks a part-time RN Case Manager to conduct SOC admissions, OASIS-related assessments, and in-home visits. You’ll manage IV infusions, collaborate with clinicians, and maintain comprehensive documentation from Sonoma County to neighboring areas.

This role requires CA RN license, BLS/CPR, and a valid CA driver's license. The position offers flexible weekend scheduling (Saturdays/Sundays, with potential for additional weekdays) and a

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting SOC admissions and assessments (OASIS desired).
• Clinical knowledge in home health care.
• Experience with high acuity patients preferred.
• Experience with interdisciplinary teams desired.
• Case management fundamentals desired.
• IV medication infusions experience preferred.
• IV therapy proficiency a plus.

Responsibilities

• Perform Start of Care (SOC) admissions and OASIS assessments.
• Conduct follow-up visits and evaluations in assigned territory.
• Make home visits to patients.
• Reconcile medications, wound care, treatments, and procedures.
• Monitor patient conditions and report changes to the clinical team.
• Collaborate with physicians and staff to deliver quality care.
• Initiate and administer IV medications safely per protocol.
• Document observations and interventions in the patient record.
