21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager to serve the Greater Sacramento area. You will perform SOC admissions, in-home assessments, and manage IV infusions, with autonomy and flexible scheduling.

Ideal candidates hold CA RN licensure, BLS/CPR, and experience with SOC/OASIS, high-acuity patients, and interdisciplinary teams. Comprehensive benefits, mileage reimbursement, and IPad provided for field work.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting assessments and Start of Care (SOC) – OASIS is highly desired.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• Previous experience with IV Medication infusions highly preferred.
• Proficiencies initiating intravenous (IV) therapy.
• Self-Directed, Empathetic, Critical thinker, Outstanding interpersonal skills, Leadership, Organized.

Responsabilités

• Conduct SOC admissions and OASIS work to determine home care needs.
• Perform follow-ups, evaluation visits, and initiate home visits within territory.
• Reconcile medications, wound care, treatments, and procedures.
• Monitor patient conditions and report changes to the clinical team.
• Collaborate with physicians and agency staff to deliver high-quality care.
• Document observations and interventions clearly and accurately.
• Administer IV medications safely per established protocols.
