Qualifications

• CA RN license required and active.
• BLS/CPR certification required.
• Experience with Start of Care (SOC) - OASIS is highly desired.
• Valid CA Driver's License.

Responsibilities

• Conduct SOC admissions and follow-up visits within Sacramento County.
• Perform home visits and evaluate patient needs in home health settings.
• Manage medications, wound care, treatments, and procedures.
• Document observations and interventions accurately.
• Collaborate with physicians and agency staff to ensure high-quality care.
• Initiate IV medications safely per protocol.
