Job summary

21st Century Home Health Services (21HHS) seeks a part-time Registered Nurse Case Manager to support patients in the Galt to Elk Grove area, visiting homes on Saturdays, Sundays, and three weekdays. You will conduct SOC admissions, follow-ups, and IV infusions, ensuring clinical excellence across a defined territory.

Qualifications include CA RN license, BLS/CPR, and strong evidence of home health experience. Join a team with growth, support, and outstanding patient outcomes.

Qualifications

• Current and unrestricted CA RN license.
• BLS/CPR Certification required.
• Experience with SOC/OASIS is highly desired.
• Experience with high acuity home health patients is a plus.
• Excellent interpersonal and leadership skills.

Responsibilities

• Perform Start of Care (SOC) admissions and SOC-OASIS.
• Conduct follow-ups and evaluation visits within territory.
• Make home visits to patients in designated territory.
• Manage medications reconciliation, wound care, and treatments.
• Monitor patient conditions and report changes to clinical team.
• Collaborate with physicians and agency staff to ensure quality care.
• Document observations and interventions accurately.
• Initiate IV therapy safely according to protocols.
