Job summary

21st Century Home Health Services in Northern California is seeking a Part-time Registered Nurse Case Manager to cover the Placerville to Cameron Park territory. Expect Saturday/Sunday full-day shifts, with flexibility for additional weekdays.

You’ll perform SOC admissions, follow-ups, and home visits to provide high-quality home health care. You will collaborate with an interdisciplinary team, document care, and coordinate IV therapies, wound care, and medications.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience with Start of Care (SOC) - OASIS is highly desired.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly.desired.
• IV medication infusion experience is a plus.

Responsibilities

• Conduct SOC Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
