Job summary

21st Century Home Health Services is seeking a Part-time Registered Nurse Case Manager for the Yountville, CA area. You will conduct SOC assessments, manage home visits, and coordinate with the clinical team to deliver exceptional patient care in a home health setting.

Qualifications include a CA RN license, BLS/CPR, and driver’s license. This role requires visiting patients in their homes and may offer an expanded schedule including weekend shifts or full-time options as described.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting SOC/OASIS assessments is highly desired.
• Experience with IV medication infusions is a plus.

Responsibilities

• Perform Start of Care (SOC) Admissions and OASIS documentation.
• Conduct follow-up and evaluation visits within the territory.
• Make home visits to patients in designated territory.
• Reconcile medications, perform wound care and other procedures as assigned.
• Monitor patient conditions and report changes to the clinical team.
• Collaborate with physicians and staff to deliver high-quality care.
