21st Century Home Health Services (21HHS) is seeking a Part time Registered Nurse Case Manager to cover Placerville to Cameron Park. You will conduct SOC admissions (OASIS), perform follow-up visits, and make home visits within your territory.

The role requires a CA RN license, BLS/CPR certification, and a CA driver’s license. IV therapy and high-acuity home health experience are highly valued. Join a team focused on quality care and patient-centered outcomes.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting SOC – OASIS is highly desired.
• High level of clinical knowledge in home health care.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Conduct follow ups and evaluation visits in designated territory.
• Make home visits to patients in designated territory.
• Reconcile medications, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to the Clinical team.
• Collaborate with physicians and agency staff to deliver the best care.
• Document observations, interventions, and evaluations related to patient care.
• Initiate and administer IV medication infusions safely per protocol.
