Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and provide direct and indirect patient care in patients' homes as part of an interdisciplinary team. You will perform skilled nursing assessments, wound care, PICC line care, and IV infusions while managing care plans across multiple counties.

Qualifications

Current RN license in good standing
BLS certification
Driver's license and auto insurance
1 year of nursing experience or more

Responsibilities

Coordinate and provide direct and indirect patient care in home settings.
Perform skilled nursing assessments and wound care (including wound VAC, PICC line care, IV infusions).
Complete OASIS/485 changes within 24 hours and ensure accurate documentation.
Supervise Home Health Aides and coordinate care with interdisciplinary team.
Communicate with physicians, care coordinators, and insurers to plan and monitor care.
