Job summary

Carle Health is seeking an experienced Home Care RN/Field Case Manager to join our interdisciplinary team. This role focuses on coordinating care plans, educating patients and families, and performing skilled nursing assessments across our home care services.

Responsibilities include OASIS/485 changes, supervising Home Health Aide visits, and collaborating with physicians, care coordinators, and insurers to ensure seamless transitions and high-quality patient care.

Qualifications

• Active IL RN license and BLS certification.
• Proven ability to coordinate care plans and document per policy.
• Experience with home health care and patient/family education.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process to meet patient needs.
• Complete OASIS/485 changes within 24 hours of receiving them.
• Supervise Home Health Aide visits and coordinate care with the health care team.
• Communicate with MD and care coordinators regarding patient updates and condition changes.
