Job summary

Carle Health is seeking a Home Care Registered Nurse/Field Case Manager to serve patients in the Illinois home health program. You will coordinate care plans, perform skilled nursing assessments, and educate patients and families while working within an interdisciplinary team.

Responsibilities include complex wound care, PICC line care, catheter care, and equipment management. OASIS changes, documentation, and care coordination with physicians are essential.

Qualifications

• Active IL RN license or eligibility
• BLS certification within 30 days
• Driver's license and proof of auto insurance
• 1 year nursing experience or more

Responsibilities

• Coordinate and provide skilled nursing care in patients' homes
• Complete OASIS/485 changes within 24 hours of admission
• Supervise Home Health Aide and coordinate care with the health care team
• Communicate with MDs, care coordinators, and insurers as needed
• Document admission, transfers, and discharge notes in policy-compliant manner
