Job summary

Carle Health is seeking a Home Care Registered Nurse/Field Case Manager to join their interdisciplinary home care team. Responsibilities include coordinating care plans, performing skilled nursing assessments, and educating patients and families across a broad home-care footprint.

Responsibilities include OASIS changes, wound care, PICC line and catheter care, and collaborating with the care team to ensure timely documentation and high-quality patient outcomes.

Qualifications

• IL RN license and BLS certification required.
• 1 year nursing experience preferred.
• Valid driver’s license and proof of auto insurance.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform complex wound care and central line care per agency protocol.
• Complete OASIS admissions, recertifications, resumptions, and discharges within policy.
• Collaborate with the interdisciplinary health care team and manage caseload.
• Educate patients and families and monitor health conditions to promote independence.
