Job summary

Carle Health is hiring a Home Care Field RN to coordinate and deliver skilled nursing care across designated communities. You will educate patients and families, perform wound care, manage catheters and IV infusions, and collaborate with an interdisciplinary team to ensure optimal outcomes.

Responsibilities include completing OASIS/485 changes, ensuring accurate documentation, and providing supervision to Home Health Aide staff as needed.

Qualifications

• Active IL RN license required.
• BLS certification within 30 days of hire.
• Valid driver’s license and auto insurance.
• Minimum 1 year nursing experience in home care or relevant setting.

Responsibilities

• Coordinate and provide direct/indirect patient care using nursing process.
• Perform complex wound care and catheter/vascular care per plan.
• Complete OASIS/485 assessments within required timelines and ensure proper documentation.
