Job summary

Carle Health is seeking a Home Care Registered Nurse/Field Case Manager to coordinate and provide skilled nursing care across multiple counties. You will educate patients and families, perform complex wound care, monitor PICC lines and catheters, and collaborate with an interdisciplinary team to ensure high-quality home health services.

Qualifications

IL RN license required and BLS within 30 days of hire.
Driver's license and proof of auto insurance.
College diploma in Nursing; 1 year nursing experience preferred.

Responsibilities

Coordinate and provide direct and indirect patient care using the nursing process.
Complete admission documentation and OASIS/485 changes within 24 hours.
Perform complex home care tasks (wound care, PICC line monitoring, catheter care).
Supervise Home Health Aide and coordinate care with interdisciplinary team.
Communicate with MD/care coordinator and document visits and orders.
