Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and provide skilled nursing care in patients' homes across the designated service area. You will work within an interdisciplinary team to assess needs, plan and implement care, and monitor outcomes.

The role emphasizes wound care, IV infusions, catheter management, and comprehensive case management with strong emphasis on documentation and compliant care planning.

Qualifications

• Current IL RN license and BLS certification required.
• College Diploma in Nursing required; 1 year nursing experience preferred.

Responsibilities

• Perform complex wound care including wound VAC, ostomy care, and dressings.
• Manage PICC lines and Hickman catheters; assist with blood draws and IV infusions.
• Coordinate care plans and discharge planning with the interdisciplinary team.
• Complete OASIS/485 assessments and changes within 24 hours of admissions or transfers.
• Document and communicate care plans, orders, and changes to MD, care coordinators, and family.
• Supervise Home Health Aide activities; assign tasks and provide written instructions.
